**Position Summary**

The Remote LVN supports digital health operations by reviewing and managing clinical documentation within a telehealth environment. This role focuses on data accuracy, chart review, and escalation of clinical concerns rather than direct patient care. **Key Responsibilities**

+ Audit and abstract patient data for accuracy and completeness
+ Review remote patient monitoring data (vitals, glucose levels, weight, etc.)
+ Document findings and escalate clinical concerns as appropriate
+ Collaborate with providers to ensure timely care plan updates
+ Maintain compliance with HIPAA, URAC, and NCQA standards
+ Participate in quality assurance and documentation improvement initiatives
