Position Overview
Certification Details- LMSW (Licensed Master Social Worker)
Job Details- Provide social services to patients and their families at OU Health under general supervision.
- Conduct discharge planning evaluations and psychosocial assessments.
- Counsel patients and others regarding diseases, treatments, social or financial situations, emotional stress, and other problems.
- Provide crisis counseling and intervention for high social risk cases including child abuse or neglect and physical abuse.
- Act as liaison between patients/families and hospital staff and other agencies.
- Evaluate effectiveness of services and adjust patient care as needed.
- Assist in discharge planning and referrals to community services and post-acute care facilities.
