Title: Physical Therapist (PT) - Home Health / Hospice
Location: Dublin, OH
Duration: 13 weeks
Shift: Day shift, 5x8 hours (8:00 AM - 5:00 PM)
Job Responsibilities

• Provide physical therapy services to geriatric patients in a home health/hospice setting
• Manage an average caseload of 5-7 patients per day
• Complete Start of Care (SOC) visits when required
• Perform timely, accurate clinical documentation and maintain clear communication
• Collaborate with interdisciplinary care teams
• Participate in on-call rotation (approximately every 6th weekend)
• Travel within an assigned territory up to a 60-mile radius
• Adhere to professional standards, policies, and dress code requirements
