JOB SUMMARY :
Serves as a single point of contact for referring physicians, patients, and community partners to reduce barriers to needed resources Directing clients to sources of emotional, financial, administrative, or cultural support Build working relationships, solve problems and support patients as they learn to self-navigate the system QUALIFICATIONS :
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES :
