Responsibilities - To perform home visits to palliative care patients.
- Performing nursing assessment on the patients.
- Initiating nursing care plan and implementing new measures.
- To educate caregivers/families on basic nursing procedures.
- To refer patients to other professional bodies such as inpatient hospice when the need arises.
- Preparing families for approaching deaths.
- Providing family with emotional and bereavement support
Requirements - Fully registered with the Singapore Nursing Board (SNB).
- Meticulous with strong attention to detail.
- Minimum of 3 years of relevant working experience.
