This position coordinates services to families involved in Children’s Long-Term Support (CLTS) Waivers Program, Children’s Community Options Program (CCOP) and Children’s Special Needs Unit. It is responsible for completing assessments, service plan development, and service coordination for children with Development/Intellectual Disabilities, Physical Disabilities and Severe Emotional Disabilities and their families. This position also collaborates with community agencies and local school districts thorough the provision of case management services.
