Function Rehabilitation Facility Position Social Worker Introduction

A Vacancy exists for a Social Worker at the Rehabilitation Unit at Life Entabeni Hospital. The Life Rehabilitation unit is part of an expanding network of rehabilitation units within Life Healthcare. The post holder will report to the Therapy Services Coordinator and will be expected to provide holistic social work intervention.

Critical Outputs

Provide social work clinical services to all patients admitted to Life Rehabilitation Unit, including on some weekends and public holidays.

Conduct assessments of the patient, family and home environment, provide counseling services and coordinate discharge planning to patients with neurological disorders such as TBI, CVA, SCI, orthopaedic conditions or multi-morbidity conditions such as Guillain Barre, etc. in collaboration with other therapy disciplines.
