Job Responsibilities:
Participate in planning related to resident, family, and community interests. Participate in discharge planning, development and implementation of social care plans and resident assessments. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, arranging transportation, etc., as necessary, coordinating activities with other departments as necessary.
Promotes optimum transition of resident for appropriate and safe placement for discharge. Provides ongoing communication with resident and responsible persons of progress/status during stay.
