Position Overview
We are seeking a dedicated Physician to provide comprehensive medical coverage across Skilled Nursing Facilities (SNF) and Assisted Living Facilities (ALF) in the Long Island market. This role offers the opportunity to lead clinical oversight, manage medically complex patients, and reduce avoidable hospital transfers through proactive, person-centered care.
Coverage Areas: Nassau County, Suffolk County, and select locations in Queens (depending on practice site)
Key Responsibilities
Skilled Nursing Facility (SNF) / Post-Acute Care
Provide dedicated Medical Directorship with emphasis on quality and compliance
Ensure 7-day medical coverage with on-site and on-call availability
Overnight coverage handled by in-house providers for continuity and responsiveness
