RN Clinical Coordinator Manager

Palm Medical Centers is a rapidly growing company currently seeking an experienced RN Clinical Coordinator with leadership experience to join our team. The RN Clinical Coordinator Manager is responsible for supporting and overseeing the Case Management (CM) team within the Central Florida (CFL) Market. This role ensures effective coordination and continuity of care for patients across hospital and skilled nursing facility (SNF) settings, from admission through discharge. The position reports directly to the Utilization Management Department.

Key Responsibilities:

• Supervise assigned Case Managers, including monitoring monthly performance metrics and providing ongoing coaching and development
• Coordinate team coverage to ensure continuity of operations
