**Description**

_Seeking an experienced nurse, preferably ED experience, to collaborate with the team and facilitate continuity of care for discharged ED patients._

_Monday through Friday, Some weekends 4 to 5 hours per day mainly afternoon/evening hours._

_Key responsibilities include_

+ _Review lab results in a timely manner, prioritizing critical findings and working closely with physicians and healthcare providers to address results_
+ _Provide detailed follow up care by reporting lab results and timely interventions to discharged patients_
+ _Educate patients on their lab results, including necessary follow up care, medications as applicable_
+ _Document results and communication in electronic health record (EPIC)_
