Primary Care Physician - San Diego CONREP

The CONREP program connects members to proper resources, safe housing, and achieving wellness goals. Each member will have psycho-educational, peer/family support, skill-building groups, and access to medication management services.

Primary Care Physician - Up to 10 hours per week (Flexible on days and schedule, hybrid position)
