Role Overview

The Primary Care NP provides comprehensive medical management to residents within Assisted Living Program (ALP) facilities. Unlike a traditional clinic, this role brings the Doctor’s Office to the resident. You will manage a dedicated panel of patients across multiple sites in the Valley Stream area, focusing on treating in place to reduce unnecessary ER transfers and improve the quality of life for the geriatric population.

Key Responsibilities

• Clinical Management: Perform comprehensive physical exams, diagnostic screenings, and acute sick visits at the patient’s bedside within the ALP.

• Chronic Disease Oversight: Manage complex geriatric conditions including COPD, CHF, Diabetes, and Cognitive Impairment (Dementia/Alzheimer's).
