The ideal candidate will provide high-quality, patient-centered care to individuals in their homes, helping them regain mobility, improve physical function, and enhance their overall quality of life.

Key Responsibilities

Conduct comprehensive physical therapy evaluations in patients’ homes

Develop individualized treatment plans based on patient needs and goals

Implement therapeutic exercises, manual therapy, and mobility training

Monitor patient progress and adjust treatment plans as needed

Educate patients and family members on exercises, safety, and injury prevention

Maintain accurate and timely clinical documentation in compliance with state and federal regulations
