About the Opportunity

Under the supervision of the Manager of Clinical Pharmacy Services and the Clinical Pharmacy Coordinator the Pharmacist is responsible to ensure safe and effective drug therapy optimizations and management for hemodialysis patients. The Renal Pharmacist participates in all aspects of care for renal patients including immunizations, prescribing/deprescribing, pharmacokinetic monitoring, and is a steward for all appropriate medication use.
