Your Opportunity:

The Pharmacist I will provide direct patient care in accordance with the AHS Pharmacy Services clinical expectations standards at the UAH, Stollery or MAHI. This includes managing medication therapy for AHS patients collaboratively with other healthcare providers in a patient centered model to ensure that medications are used safely, effectively and appropriately. You will assess patients for potential or actual drug-related problems and resolve or prevent drug-related problems to optimize patient outcomes. Actively participating in patient care rounds where applicable. Liaising with members of the health care team and developing professional relationships with the patient and their family to formulate a medication treatment plan.
