Position Overview
Job Responsibilities - Lead and supervise pharmacy teams to ensure safe, efficient, and compliant medication dispensing and supply operations.
- Oversee medication reconciliation, pharmaceutical care planning, and resolution of medication-related issues to optimize patient outcomes.
- Provide expert medication advice, counselling, and education to patients, caregivers, healthcare professionals, and the public.
- Collaborate with multidisciplinary teams to develop evidence-based, cost-effective treatment plans and ensure appropriate medication monitoring.
- Support discharge planning and medication adherence initiatives, including patient education and self-medication programs.
