As a member of the team, a primary function will be to provide short‑term supports to individuals returning from hospital or to prevent admission, in addition to other aspects of service delivery such as assisting individuals who have fallen but are uninjured and responding to telecare alerts when individuals activate their pendant alarm or when no movement is detected in their home.
