Summary:


Provide nursing care in physician practices, duties can vary by practice specialty. Carry out instructions provided by clinical coordinator(s), RN Practice Manager, clinical supervisors and Director, Clinical Services of Physician Practices.

Responsibilities:
+ Patient navigation through scheduled pre-operative testing; post-operative care at home
+ All Structural Heart Program registries management
+ Referral Management
+ Patient triage calls
+ Structural Heart statistics
+ Communicate effectively in both oral and written form, as appropriate for the needs of the audience. Assist departments with questions.
