This role focuses on delivering high-quality OT interventions to support individuals in living safely and independently at home and to facilitate timely hospital discharge.
What will your responsibilities be?
Assess individuals receiving care packages to ensure appropriate OT support.Provide interventions that promote independence and safety in the home.Support hospital discharge planning through timely assessments and recommendations.Conduct assessments related to single-handed care, moving and handling, equipment, and adaptations.Build strong working relationships with service users, families, and professionals.Maintain accurate records and contribute to multi-disciplinary planning.
