General Responsibility : Tri-County Community Action Agency is currently seeking full-time Nurse Practitioners to provide primary care services in our Federally Qualified Health Center (FQHC). Primary Care Providers (PCP) will work collaboratively with other team members in providing high-quality, comprehensive primary medical care to Tri-County patients using best practice and evidence-based standards. Tri-County is an NCQA recognized Level III Patient Centered Medical Home and as part of the team-based approach to care, the PCP will participate in coordinating care for patients, support patients and families in self-management and will engage in continuous quality improvement activities aimed at specific patient population needs.
