The Homeless and High Intensity User Service at Guy’s and St Thomas’ is an award-winning team delivering compassionate healthcare to people experiencing homelessness. We are a supportive multidisciplinary service committed to reducing health inequalities and improving access to care for some of the most vulnerable members of our community.

We are seeking an experienced Band 7 Adult Nurse Practitioner to join the team on a one year secondment.This is a unique opportunity to provide high-quality, person-centred nursing care to individuals who are rough sleeping or at risk of homelessness across Lambeth, Lewisham, Southwark and the City of London. This position will be predominantly based and working with colleagues in the City of London sites.

The role is primarily community-based, working in non-NHS settings including hostels, day centres and street outreach. You will also attend the team base at Gracefield Gardens when required.<...
