Nurse Practitioner Specialty: Family Medicine Location: Portage, IN Education/Experience Requirements: - Graduate of an accredited Nursing program; Masters of Nursing degree required
- Minimum of 3 years nursing experience required; 5 years preferred
- Licensed Registered Nurse in the State of Indiana
- Prescriptive authority privileges in State of Indiana
- Board Certification is required to be granted necessary hospital privileges for employment
- CPR certification required
- Conveys positive attitude and image internally throughout Community Healthcare System and externally within the community
