Description
As part of an adult moderate CCBHC team, Nurse Care Manager will provide the following:
Serve adult clients, age 18 and older, that are diagnosed with a severe mental illness, substance use disorder, or co-occurring disorder.Combination of office and/or field-based services with potential transportation of clients using agency or personal vehicles as determined by needs of consumers and/or program.Meet clients where they are in terms of current mental health symptoms and treatment preferencesProvide med reconciliationCoordinate care and communicate freely with consumers’ primary care providersProvide nursing assessment and wellness services for consumers as appropriate.Provide follow up and coordination from hospital or ER visits.Work collaboratively with community partnersProvide services in...
