**About this role:**

As a PRN Nurse Practitioner in Palliative Medicine you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In this role you will make home visits to help patients set goals of care, manage symptoms and medications, and coordinate care.

**Schedule:**

+ **1–2 day shifts per month** Monday–Friday, 8:00 AM – 5:00 PM
+ **1–2 overnight call shifts per month** Monday–Friday, 5:00 PM – 8:00 AM
+ **1 weekend call shift every 5–6 weeks**

**What you’ll bring:**

+ APRN from an accredited program
+ 1-2 years of experience in Hospice and or Palliative Care, Required
