Overview


The Patient Care Manager Assistant supports the Clinical Manager in coordinating staff to ensure continuity and quality of care for assigned home health patients. Ensures team members are informed, skilled, and deliver care efficiently in compliance with physician orders and regulatory guidelines.


Responsibilities


• Respond to phone inquiries from field staff, physician offices, referral sources, and patients; take messages and orders as needed.• Monitor incoming faxes and relay information to appropriate personnel.• Coordinate communication and transmit relevant documents to physician offices.• Track pending lab results, facilitate follow-up, and communicate findings to physicians and relevant personnel.• Monitor hospitalized patients to support effective care coordination.• Support Clinical Manager in managing software system reports to enhance care coordination an...
