Purpose of Position: Under the direction of the Palliative Specialist and within the Agency policies and procedures, the LPN follows the plan of care on patients with a serious or life-limiting illness within the guidelines of his/her licensure to provide for the safety and quality of care treatment or services.
Description: Provide palliative care services including symptom management, education on medications and lifestyle changes to enhance quality of life, while collaborating closely with palliative specialist and community providers. Work with MSW to initiate discussions on advanced directives and goals of care to ensure a smooth and timely transition to hospice. Assesses patient and family needs, reports new or pertinent findings to the Palliative Specialist RN and MD.
