Dates of Coverage: Full time starting 9/2025 Reason for Coverage: open slot Practice Setting: Hospital Based Will the Locum Tenen Physician be Required to Supervise CRNAs?: both Patient Age Demographic: Geriatric Number of Operating Rooms: 8 Facility will reimburse for the following costs: Flight, Hotel, Rental Car Procedures Required:

• Spinal Anesthesia
• Epidural Anesthesia
• Regional Anesthesia

Submission Requirements

• Board Certified - REQUIRED
• Active NY State License (Will accept those willing to obtain)- REQUIRED
• Active BLS, ACLS, PALS - REQUIRED
• $25K perm cap - REQUIRED
