- Two Supervisor References
- BLS/CPR
- Immunization records (PPD, Flu and Physical) if updated
- Covid Vaccination Card
Requirements: - State LPN license
- BLS/CPR preferred
- Min 1 -2 year of LPN experience in rehab or skilled nursing facility will be preferred .
- Duties include taking Vitals, assisting with ADL's, transportation, Medicine administration and reminders, Assisting RN's.
- Care/Rehab or Skilled Nursing Facility is preferred
