Position Overview:

• We are seeking a compassionate, dedicated, and bilingual (Arabic/English) Licensed Practical Nurse (LPN) to join our Value-Based Care team.


• This role is specifically focused on supporting patients enrolled in our Chronic Care Management (CCM), Principal Care Management (PCM), and Remote Patient Monitoring (RPM) programs at our Medical Center.
• The ideal candidate is a proactive, patient-centered clinician who thrives in a care coordination environment and is committed to improving outcomes for our chronically ill patient population.
