Position Summary
Provide direct services to patients as part of a multidisciplinary team. Provide ongoing, individualized care to patients in the inpatient unit including discharge planning.
Essential Duties & Responsibilities
Direct patient contact:
* Completes social work/psychosocial assessments on newly admitted patients. Reviews the assessments completed by the other inpatient social workers.
* Ensures goals and an individual plan of care are established for patients by the inpatient social workers.
* Collaborates with individuals, families, health system and community based interdisciplinary professionals on the plan of care.
* Ensures the timely development of the discharge plan for patients by the inpatient social workers. Assist with complex discharges.
