*Purpose of the position:*The Knapp Rehabilitation Center Admission Liaison exists to grow and facilitate the admissions of individuals to the Knapp Rehab Center both within Hennepin Healthcare and with our external partners. This position is the face of Knapp Rehab for internal and external referring organizations, predominantly, acute care hospitals throughout the Twin Cities area. The Liaison must build productive professional relationships and is often the first impression patients, families and referral sources experience with Knapp Rehab. This position serves as a link between referral sources (both internal and external) with the goal of efficiently and effectively transitioning individuals into their inpatient rehabilitation episode of care.
