Parkdale Queen West Community Health Centre (PQWCHC) is a dynamic, community-based health care organization that has historically served mid‑west downtown Toronto, and now provides outreach services across the entire city. The Homelessness and Addiction Recovery Treatment (HART) Hub Nurse Practitioner is responsible for providing health care and treatment for clients who use substances. The NP is part of the HART Hub services and will work closely with the mental health, substance use care (continuum of treatment, recovery, harm reduction), and health promotion teams to ensure cross‑team and cross‑site collaboration.
