The Physical Therapist consults, evaluates, plans and administers skilled Physical Therapy services prescribed by a physician to homebound patients to restore function, relieve pain and prevent disability following illness, disease, injury, or surgery. Collaborates with Inter-disciplinary team members to assure patient's needs are met, and quality of care is achieved.
