Position: HIV & Chronic Diseases Care Coordination Team Leader

Location: Centurion, Gauteng

Position Purpose: As the HIV & Chronic Diseases Care Coordination Team Leader, you will lead a multidisciplinary team responsible for coordinating care for beneficiaries living with HIV, diabetes, hypertension, cardiovascular disease, mental health conditions, respiratory diseases, and other chronic conditions.

You will oversee clinical and operational quality, mentor and develop the team, manage complex escalations, strengthen provider relationships, and ensure compliance with clinical guidelines and organisational standards. Your leadership will directly contribute to improved health outcomes, enhanced beneficiary experience, and effective risk management.

Experience & Qualifications :

Registered Nurse (3- or 4‑year qualification) OR health sciences degree — mandatory
