Role Summary

We are looking for a friendly and dynamic nurse to join the Haemoglobinopathy team at Sandwell and West Birmingham NHS Trust as a liaison nurse. The role involves facilitating the transition between paediatric services and bridging the gap between hospital and home.

Responsibilities

• Screening and counselling on haemoglobinopathy diagnosis
• Clinically support patients in outpatient and inpatient settings, delivering day treatments and managing unwell patients
• Coordinate and participate in regional multidisciplinary teams (MDTs), teach and raise awareness within hospital and community
• Visit patients in their own homes to deliver a holistic approach to care

Requirements

• Full UK driving licence and access to a personal vehicle
• Experience caring for people with haemoglobinopathy conditions or awareness of their challenges
