Key Responsibilities

* Provide direct support to families in home and community settings

* Assist clients in developing parenting skills, daily living skills, and coping strategies

* Conduct needs assessments and develop tailored support plans

* Facilitate access to community services, healthcare, housing, and education resources

* Monitor client progress and maintain accurate case notes and reports

* Work collaboratively with internal teams and external stakeholders

* Ensure all services are delivered in line with relevant legislation, policies, and ethical standards
