Lively Medical Center is seeking a Family Nurse Practitioner (FNP) to join our established outpatient primary care team. This is a full-time, outpatient-only position offering a consistent schedule and strong clinical support.
The ideal candidate is committed to delivering comprehensive primary care across the lifespan and values working within a collaborative, team-based model. Highlights:
* Outpatient-only practice
* Schedule: 7:00 AM 5:00 PM, 4 days per week.
* Patient Volume: patients per day
* Call: Rotating call schedule shared among 10+ providers
* Supported by dedicated clinical staff (CMA/LPN) for rooming and patient flow
* Provide comprehensive primary care services including:
* Routine wellness visits
* Medicare Annual Wellness Visits
* Acute care visits
* Hospital follow-ups
* Preoperative examinations
