The team provides physician coverage across sites of care, to ensure safe transitions, and aid in the appropriate placement of patients in Skilled Nursing Homes, Assisted living facilities, Independent living facilities or to home with support services. We are seeking physicians with strong interpersonal and leadership skills, with a demonstrated understanding of effective care planning and management for improved quality outcomes.
