Location: Schaumburg, Illinois, United States (Float across Chicago metro: Chicago, Evanston, Brookfield, Oak Park)

Pay Rate: $74.87/hour
Hours: Average 30–35 hours/week
Schedule: 3 x 10-hour shifts during weekdays + every other weekend requirement

• This is a Family Nurse Practitioner (FNP)-required role delivering primary and family-focused care across a multi-site outpatient network
• Providers rotate across 13 clinics based in the Chicago metro area
• Care is delivered in collaboration with a dedicated clinical care team focused on panel management and outcomes improvement
• Role reports to the Practice Manager
