Under the supervision of the Director of Quality and Patient Safety, the Disease-Specific Care Coordinator functions as an expert nurse in the disease specific field, resource for nursing staff and patient care in this area, consultant, researcher and outcomes focused specialist. The Coordinator is responsible for providing coordination and oversight for the assigned hospital's disease-specific program. This multidimensional role will focus on current treatment, prevention, education to improve quality of patient care. The Coordinator will drive current certification through performance improvement activities and standard compliance.
