This will include supporting people to achieve their individual goals and maximise independence, choice, control and decision-making

The ideal post holder would have experience of; –
~ Working with, and the ability to support individuals who can challenge services
~ Responding to the emotional needs of individuals in a way that is positive and conducive to their wellbeing
~ Working in partnership with management, MDT’s and other professionals to develop and review individuals Positive Behaviour Support Plans
~ Contributing to, and following Positive Behaviour Support Plans and maintaining appropriate individual records
