Sentara Rockingham Memorial is seeking a Community Health Worker- Outreach

This is for the Hand-in-Hand Virginia Resource Mothers Program - Home Visiting program for newly pregnant and parenting teens.

Works in collaboration with healthcare team members to support diverse populations by facilitating access to essential health services, equipment, and resources that promote health. Provide education on key health topics, support behavior change, screen for risk factors, and partner with community organizations to deliver effective outreach and programming. Help individuals navigate public benefits, connect with support services, and promote positive health outcomes through coaching, education, and early intervention.
