Position Overview
Responsibilities - Patient Assessment: Evaluate patients’ mental, emotional, and social needs, including family dynamics, financial situation, and support systems.
- Care Planning: Develop and implement care or discharge plans in coordination with doctors, nurses, and other healthcare staff.
