Education

* Bachelor's Degree Pharmacy Required Or

* Other Pharm.D. Required And

* Other - CLIAFE Must comply with CLIA regulations regarding foreign education equivalency Required

Experience

* None required and

* 3 Years hospital experience or residency Preferred

Licenses and Certifications

* PHAR - Pharmacist Upon Hire Required
