About the Role


We seeking a Per Diem Clinical Pharmacist to join our dynamic, patient‑centered care team. In this role, the Clinical Pharmacist provides comprehensive pharmacy services within a collaborative, team‑based FQHC environment. Core responsibilities include chronic disease management, Medication Therapy Management (MTM) , provider collaboration, population‑health–driven initiatives, and education for both patients and staff. The Clinical Pharmacist also participates in interdisciplinary care team activities, supports medication safety and regulatory compliance, and contributes to dispensing and daily pharmacy operations as needed.

Key Responsibilities
