General Summary of Position
The Pulmonary Nurse Navigator coordinates care for patients with acute and chronic pulmonary conditions, serving as a clinical resource throughout diagnosis, treatment, and follow-up. This role provides patient education, symptom assessment, care coordination, and communication between patients, pulmonologists, primary care providers, and other specialists to promote high-quality, patient-centered care. The Pulmonary RN will also offer admin support by triaging portal/ phone messages, assisting the team with scheduling procedures, and seeking approvals for denied prior authorizations. In collaboration with the patient and family, provides theory-based professional nursing care and coordinates care delivery with various members of the interdisciplinary team.
