Job Summary

Diabetes Care and Education Specialist - RDN serves in an advanced role providing in-depth analysis of the patient with diabetes and providing specialized care and in-depth education for persons with diabetes and their families. Serves as a team member in assessing needs, planning and providing education and care, evaluating outcomes and coordinating follow up care for persons with diabetes.
Job Duties


+ Assesses patient's appropriateness for intensive insulin therapy.

+ Provides diabetes self-management education and training which includes evaluation of the individual’s skills, support, and barriers to care.

+ Assess general diabetes knowledge, evaluate physical findings and anthropometric measurements,

+ Teach person appropriate essential self-care behaviors to improve effective self-management of their disease. These may include healthy eating, being physically active, monitoring of blood sugar, medica...
