Case Manager

Position Summary

Under the supervision of the Case Manager Supervisor, Clinical Supervisor and Director you will be responsible to work collectively within a team environment ensuring all families' needs are met in a timely manner, following the practices and processes that support the mission, vision statements and goals of Makoon Transition Inc.

Responsibilities

• Oversee families residing in Makoon Transition Inc., engaging them to build and meet their identified healing goals. Help them work alongside Child Family Services of reunification and assist in navigating the child welfare system.
• Be prompt and ready for work in a positive manner.
• Sign in and out daily for accountability of attendance and tracking of time.
