EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated full time Case Manager to join our Community Homebase Program.


+ Wage: $26 per hour

+ Schedule: Monday through Friday 9am to 5pm at least 1 flex day a week

+ Location: 320 Carleton Ave in Central Islip, NY




What drives the core purpose of this job:

The Case Manager is responsible for all aspects of Home-based Crisis Intervention and case management services for client families in the Home Base (Community) Program.
